In this in-theatre live surgery video, we take you into the operating theatre to watch Dr Dave Lee, Head & Senior Consultant, Division of Sports, Shoulder & Elbow Surgery, Department of Orthopaedic Surgery, National University Hospital (NUH) Singapore and his team perform an anterior cruciate ligament (ACL) reconstruction surgery.
It is a surgery procedure done under general anaesthetic.
Before the surgery, an MRI scan of the knee will help in the diagnosis of the ACL tear. The surgeons will also examine the patient under general anaesthesia to assess the amount of knee laxity, using the pivot shift test and Lachman’s test.
Steps of the surgery:
- The quadriceps tendon is harvested and prepared.
- Using an anterolateral portal, the knee arthroscopy is started. The knee is examined systematically to look for and confirm the ACL tear as well as for injuries to the meniscus and cartilage.
- The femoral footprint of the ACL is prepared and any remnant ACL is removed to prevent interference with the graft passage.
- The femoral tunnel is drilled first followed by the tibia tunnel.
- The graft is passed through the tunnels and the cortical button flipped.
- The knee is cycled to stretch the graft and ensure the button is seated well on the cortex.
- The graft is fixed on the tibial end with a button.
- The knee is taken through a range of motion to ensure there is full extension and flexion.
- A lateral extra articular tenodesis is performed to further stabilise the knee to protect the ACL graft.
