Chris and Sam sat down with Taylor Goldberg to discuss hypermobility spectrum disorders, hyper-mobile Ehlers-Danlos Syndrome (also known as hEDS), and upper cervical instability. We initially dive into an article published earlier this year by Leslie Russek and colleagues titled, “Presentation and physical therapy management of upper cervical instability in patients with symptomatic generalized joint hyper mobility: International Expert Consensus Recommendations.” Within the paper, we talk about diagnostic criteria, screening for red flags, the clinical examination, and recommended treatment interventions. Taylor discusses the outdated terms and beliefs that still occur in research and clinical practice surrounding things such as chiropractic subluxations vs true subluxations, posture, and alignment. We then discuss medical gaslighting for hEDS along with examples of both under and over-medicalization of this condition. We then discuss strength training for this population, and Taylor finishes the discussion by dispelling a few more myths surrounding those diagnosed with hEDS.

A unique part of the subjective exam that’s different from an orthopedic exam is getting details on their bowel and bladder function including diet, fluid intake, and frequency of emptying as this helps us understand dietary and lifestyle contributions to their symptoms. We spend a ton of time talking about this before the objective exam.
Reasons that I may be sending an individual back to their physician for further assessment prior to further treatment would include signs of an active infection that hasn’t been treated, someone with unmanageable pain that may be appropriate for pain medication (this would include a vaginal suppository of Valium or other medications for example), a significant prolapse that does not reduce, and skin changes that potentially medical management. Red flags include sudden changes like someone who is suddenly losing a large volume of urine or leaking feces, sudden neurological changes like progressive weakness or sudden sensory changes, excessive bleeding. If it’s someone pregnant, someone with high blood pressure, changes in fetal movement, any signs of vaginal bleeding during pregnancy, sudden SOB or signs of DVT. I’m sure there are more. Of course anyone not progressing as expected will be referred back to their treating physician.

Her work has broad reach, extending from clinic to system. Maxi draws on her experiences as a physical therapist to probe the factors that influence relational aspects of care. Her conceptual framework of the therapeutic relationship in physical therapy is being used in research with uptake in entry-to-practice and post-graduate education and clinical practice. Maxi also works to advance the nascent area of research impact assessment. She has collaborated with local, national, and international health system stakeholders to co-develop and implement frameworks to assess research impact on informed decision-making in health services and policy.
