An Interview with a Holistic Pediatric Dentist

Ok the title is a bit of a click-bait, but I did have the pleasure of interviewing Dr. Staci Whitman on the podcast. Dr. Whitman is a practicing pediatric dentist in Portland, Oregon. She is a "functional" pediatric dentist who practices in a demographic that caters to more natural and holistic modalities. Her practice is heavy in dietary guidance, vitamin supplementation, ozone therapy, tie/lip tie release, myofunctional therapy, hard tissue lasers, and also traditional SSCs and general anesthesia.

I wanted to have this conversation to widen my horizons and provide me with more context to educate and converse with my own patients who have concerns about fluoride, toxins, and materials. I was surprised to find that I agreed with the majority of points that Dr. Whitman discussed. You can tell that she's passionate about educating her patients, promoting a healthy diet, and focusing on prevention rather than picking up a handpiece to fix the world's problems. I think this will be a great episode for stimulating some critical thinking skills about the way we discuss treatment options with our crunchier families that we all know and love!

Dr. Alex Otto and her husband, Tim, are previous guests on the podcast. Dr. Alex is a pediatric dentist, they own and operate four offices in the Austin, Texas area. Tim has an MBA and a strong business background working with fortune 500 companies. They join me on the podcast to discuss their latest project, Alcan Dental Cooperative. Alcan is a cooperative model in which pediatric dentists can join in exchange for a minority equity position. Unlike DSO/PE buyouts, a dental cooperative works by helping dentists systemize and grow their practice without being forced to sell out to a majority shareholder. Today I have the pleasure of hosting my former boss, but current friend and mentor, Dr. Dennis Carreras. Dennis started and ran a large practice with four offices in central Minnesota for years before recently selling his practice equity to his partner. I worked for their office after residency prior to COVID and starting my own practice in Missouri. Sedations at the Mexican Border and Going All In on Bitcoin

Joe Sparacino is a general dentist who joins me for what may be one of the most interesting conversations I've had yet on the podcast. Dr. Joe started a large practice out of dental school down at the southern Texas border providing care to underserved kids with Medicaid. He built two large practices before selling and putting his practice proceeds into bitcoin back in 2019. He since has bought and sold four other practices and has walked away from clinical dentistry (at least for now). He has spent early retirement pursuing a passion for music, along with casually starting the company Membersy. Membersy is a dental membership technology service that has rapidly grown and is now used by many of the large DSOs.

Dr. Joe is a risk-taker, a free-thinker, and very obviously driven to create and grow his ideas into something special. He currently lives in Colorado and is working on his next idea, the Happy Patient Company. This would be a service to streamline resources and remove barriers to grow practices for dentist who are looking to achieve their goals.

A Portland Start-up Journey and Leasing vs Building, with Dr. Brandon Schwindt

Dr. Brandon Schwindt is a pediatric dentist who practices in Portland, Oregon. He started Kona Kids Dentistry from scratch in 2003, and practiced for 6 years in a leased space before making the investment and buying land to build a new office. He discusses the conversations he had with his financial team to determine if building a large office was the right move from a growth and tax perspective.

In 2011, he completed the building with 3800 sq ft and 8 op chairs plus a hygiene bay. He practices in the location with his wife, who works as a GP seeing adolescent patients, along with an additional new associate.

Dr. Schwindt and I spend a lot of time discussing the process of transitioning from a leased space into a ground-up build, and the lending and tax consequences that go along with it. We review how deductible rent expenses from the practice can go towards paying the mortgage on the building. He explains how depreciating components of the building and smart accounting work helped to lower his tax liability while building equity that he can use to boost his retirement. He also gives advice to new pediatric dentists who are trying to decide if building or leasing is the right choice for their practice.

How Good of a Pediatric Dentist are You? A Score System Breakdown

In this solo episode, I wanted to review a scoring system I created that allows pediatric dentists to reflect and compare their own attributes. Some pediatric dentists are great at running a business, some are excellent clinicians, some have great behavior management with the kids, and some are extremely efficient with their time. But often times as dentists we fail in other categories where we should be trying to improve. For example, just because somebody can see 80 patients a day and produces 2 million dollars a year doesn’t mean that they are truly improving oral hygiene in their community, that they are actually doing good dentistry, or that they have a staff that is proud to work with them. The scoring system includes the 10 categories relevant to a good pediatric dentist, with a 1-10 score for each (100 total possible points). The idea is to go through and reflect on these categories with minimal bias to determine how good of a pediatric dentist you are. The 10 scoring categories of a good pediatric dentist are broken down in this podcast and include: Clinical skills and outcomes

Impact on community oral health

Parent perceptions

Involvement in organized dentistry

Practicing with Hearing Loss and How a Cochlear Implant Changed My Life, with Dr. Jay Crews
Dr. Jay Crews is a pediatric dentist who has been in private practice for 20 years. He owns a large practice in Owensboro, Kentucky, where he lives with his wife who is also a pediatrician. Dr. Crews discovered that he had worsening and profound hearing loss during his pediatric residency. He began wearing hearing aids and dealt with progressive hearing loss until it got to the point where he was nearly unable to function. He spent his days at the office lip-reading and relying on his staff to help him work through appointments with both patients and parents. In 2022, Dr. Crews underwent surgery and received a cochlear implant in his right ear. He shares this story with us and tells us how much better his quality of life is now that he can hear again. After a decade of not being able to listen to the radio or have a phone conversation, he's listening to the Brews and Tiny Teeth Podcast on his way to work in the morning. This was a great conversation and it was a pleasure to have Dr. Crews on the podcast to share his story.

Three Start-ups in Three Years, with Dr. Alex Otto and Tim Otto, MBA

Dr. Alex Otto is a pediatric dentist who with her husband, Tim, have opened three pediatric dentistry practices since 2020. They have offices in Buda, Kyle, and Austin Texas with plans to continue expansion while providing a 5-star patient experience.

My Journey From Associate to Partnership, with Arielle Faden

Dr. Arielle Faden is a pediatric dentist and practice owner out of Jersey City, which is a large metro area near New York City. She was a guest on the podcast almost two years ago when she talked about practicing as an associate in a large city like New York. Since that time, she has gone through the journey of partnering with her boss to become a 50/50 practice owner.

We discuss how there's a lack of podcast content for dentists in these situations, but that partnerships are a great opportunity for dentists who aren't interested in either associating or going through the stresses of a scratch start. Some of the things Dr. Faden talks about:

Improve Your Posterior Zirconia Crown Game with Alphabyte

Raising Good Kids as Pediatric Dentists, with Drs. Chad and Beth Pfohl

I love interviewing a good power couple where we can have a drink and talk about teeth! Today we are joined by Dr. Beth Pfohl, a pediatric dentist, and her husband Dr. Chad Pfohl, an oral surgeon. The Pfohls are University of Iowa grads and at AAPD 2022 in San Diego, we had a few drinks at the welcome reception and I decided I needed to have them on the podcast.  Dr. Beth is unique in that she went back to dental school later in life with two kids at home, and continued for the additional two years to complete her pediatric residency. Dr. Chad is a partner at a large group practice and he shares some of his wisdom on how to find good team members and associates to bring into the practice. They come on the podcast to talk about life in the working world, as well as how to find work-life balance between running a full patient load at the office and raising good kids at home. 

As pediatric dentists, we naturally love working with kids and tend to a be a very family-oriented profession. My wife announced that we will be having our first little baby girl in May, so I thought it would make for great conversation to discuss in the upcoming podcasts how having kids influenced or changed the way that pediatric dentists practice dentistry.

Should Pediatric Dentists be Doing Ortho? With Dr. Lee Boyd

Dr. Lee Boyd is a pediatric dentist and practice owner from Allen, Kentucky, a town of just 700 people. He joins us today to talk about how even in a rural practice, he is able to generate massive revenues from the incorporation of orthodontics and Invisalign. 

We discuss how pediatric dentists are the oral-facial growth experts and that we should be doing our part to not only diagnose, but also to treat malocclusions and orthodontic problems for our patients. Dr. Boyd is an advocate for using iTero scanners to offer patients Invisalign (he starts over 100 cases a year), as well as using this technology to show parents different treatment outcomes during the mixed dentition phase. We talk about differences in treatment for both permanent as well as mixed-dentition cases, including how you can use clear aligners for expansion as well as impacted teeth in 90+ percent of cases.

Minimally Invasive Treatment Options and Pushing the Limits, with Dr. Debra Cohen

Dr. Debra Cohen is a pediatric dentist from New Jersey. She practices at Riverfront Pediatric Dentistry with Dr. Eyal Simchi. After seeing some of the recent cases on iPedo that Dr. Cohen had posted showing impressive before and after radiographs using LSTR, margin elevation, and Hall crowns, I decided it would be an interesting episode to have her on the show to discuss these techniques.

Dr. Cohen talks about how at their practice, they try to use focus on positive patient experiences and avoiding sedation by using sedative fillings with glass ionomer to assess the health of the pulp. They are able to treat and save a lot of teeth without local anesthetic, complete caries removal, or the use of general anesthesia. This was a great conversation and it was interesting to learn more about treatment philosophies that deviate from what we were trained in residency, but may very well be mainstream and supported by literature in the future.

Managing Anesthesia Emergencies, with Dr. Richard Marn, MD

Dr. Richard Marn is a dual board-certified physician in anesthesiology and pediatric anesthesiology. He trained at NYU and Johns Hopkins Hospital, and was on Mt. Sinai Hospital faculty as one of their pediatric anesthesiologists for 11 years. He works in New York providing pediatric in-office sedations for pediatric dentists and oral surgeons through his company New York Medical Anesthesia.

Dr. Marn is also unique because his wife Dr. Jessica Marn is a fellow pediatric dentist! 

Drs. Richard and Jessica Marn recognized that medical education for dentists and oral surgeons is fragmented and incomplete. They co-founded an education company, Blue Pacific Medical Simulation, that teaches dental offices how to properly respond to medical emergencies in their office.

Selling to MB2 instead of a DSO, with Dr. Jon Lindblom

Dr. Jon Lindblom is a pediatric dentist and practice owner from Bartlesville, Oklahoma. He's been a practice owner for 10+ years in his small hometown in Oklahoma, and enjoys the benefits of practicing in a busy and uncompetitive environment that a small town has to offer. Ditch the Alginate, Start Scanning Your Spacers! With Dr. Gerardo Martinez

Dr. Gerardo Martinez is a pediatric dentist who practices out of Milford, Delaware. After spending time in California, Florida, and Texas, Dr. Martinez decided to start a practice in rural Delaware. We discuss a whole range of topics on this podcast, including the differences in patient populations he has treated throughout the country. He believes the future of pediatric dentistry is bright, and he discusses how much need still exists in the more remote areas like Delaware.

Dr. Martinez has a unique practice philosophy. We talk about how he is okay with tougher/crying appointments and being aggressive with extractions while at the same time using SDF, Hall crowns, and glass ionomer products to manage decay. 

I had heard that Dr. Martinez invested in and uses a scanner to take digital impressions for his space maintenance. I hate fitting bands and taking impressions, so we took a dive into the costs and benefits of purchasing and using an intra-oral scanner in a pediatric office. It definitely changed my perspective and is likely something I will purchase in the near future.

Busting Fluoride Conspiracies, with Dr. Johnny Johnson

Dr. Johnny Johnson is a pediatric dentist from Florida who joins us on the podcast today. Dr. Johnson practiced full time as a pediatric dentist until a bicycle accident in 2002. Over the next 6-7 years he continued to practice while undergoing multiple surgeries on his wrist. He eventually hung up the handpiece when his wrist needed to be totally fused. We spend some time talking about disability insurance and what that journey looked like for Dr. Johnson.

Dr. Johnson now spends his time advocating for public water fluoridation. He is a leader in the American Fluoridation Society and fights every day all around the country and world to promote the benefits of community water fluoridation.

I try to challenge Dr. Johnson with the current evidence of fluoride, and some of the conspiracy theories and claims that anti-fluoride parents present to us. Dr. Johnson gives us the evidence and rebuttals to counter these claims.

Our State Doubled Its Medicaid Reimbursement, Here's How We Did It! With Dr. Aaron Bumann

Dr. Aaron Bumann is a pediatric dentist practicing in the Kansas City, Missouri area. He's is an active member of the Missouri Academy of Pediatric Dentistry and has been the leading voice advocating for improvements to our state Medicaid to provide better access to dental care for kids.

Dr. Bumann and I talk about his background and what his practice is like in the Kansas City area. We talk about the benefits of being an associate dentist, and how you can create a healthy professional work environment when you find the right office and surround yourself with good team members.

The focus of our conversation deals with the recent Medicaid fee increases that have been approved in Missouri. The governor recently approved a budget increase that will nearly double the current fees for dental services. To give context, a typical pediatric recall appointment that historically reimbursed around $90 will increase to around $180. This historic increase will allow more kids to have access to quality dental care in our state. Dr. Bumann discusses his role in making these fee increases happen, and gives some insight on what actions providers can do to advocate for changes in their own states.

Why Aren't More Pediatric Dentists Starting a Practice?
Even with a broad interest in practice ownership, giant student debt is negatively influencing future practice ownership. This was the summarized conclusion from a recent cross-sectional study in the AAPD journal evaluating practice ownership intentions and educational debt. On today's solo episode, I wanted to do a quick review of this article and share my thoughts. I receive inquiries several times a week from young pediatric dentists who express interest in starting a practice. I can tell you first hand that there is a lot of interest in the subject. Some dentists commit and make it happen right away, and for others it takes more time due to additional barriers. Even with the stresses of running a business, I still encourage practice ownership and I think this article does a great job shedding some light on restrictions that come with it. Shoutout to Dr. William Alexander, Dr. Joel Berg, Dr. JoAnna Scott, Dr. Roger Sanger, and Dr. Zheng Xu on publishing this study. A Cancer Diagnosis Changed the Way I Practice, with Dr. David Stanley
Dr. David Stanley is a pediatric dentist and partner at Wild About Smiles Pediatric Dentistry and Orthodontics in Murfreesboro, Tennessee. Bozeman Montana, Elk Hunting, and Hard Tissue Lasers, with Dr. Justin Stevens

Dr. Justin Stevens is a pediatric dentist and practice owner in Bozeman, Montana. His wife, Joni, is also a pediatric dentist. Together they acquired a practice and have grown it to the point of constructing a brand new 10-op building. I invited Dr. Stevens to be on the podcast after spending some time in Bozeman after my annual September elk hunt. My Overhead is 29%... Here's How I Do It

As the two year anniversary of opening my practice approaches, I thought it would be a fun exercise to go over my profit-and-loss statement from the last 12 months and breakdown my expenses. The average overhead in pediatric dentistry is around 50%, but I think we can do better. 

I've been able to run a very low overhead, lean office by keeping my expenses in check and doing many services myself. In this episode, I review some of the following that allow me to keep a sub-30% overhead:

- How to calculate a true overhead (what do I include and what do I take out).
- How being rural keeps my rent, staff salaries, and advertising budgets low
- Bringing in more revenue is an easy way to lower overhead that doesn't involve cutting costs
- Save money by doing your own bookkeeping, website, marketing, payroll, etc.
- Where to shop for dental supplies, and why the big suppliers can eat into your profits.

Interview with a Medicaid Audit and Compliance Officer

In response to the recent popular episode with Dr. Roy Shelburne, I decided to reach out directly to my local state Medicaid office to talk to somebody about how criminal medicaid cases are handled. Although I wasn't able to convince any members of the department to join me on the podcast directly, I was able to take and share notes from the conversation. There was a lot of listener feedback with confusion about how a dentist could end up in prison over simple billing and coding errors. I was able to shed a lot of light on that in my conversation with the Medicaid auditor. 

I also give my thoughts on a recent Medicaid fraud/kickback scheme involving the indictment of two individuals at a Texas pediatric dental office involving 6 million dollars worth of fraudulent and criminal dental billing to Medicaid (allegedly).

Should Your New Practice Take Medicaid? With Dr. David Knight

I Was Arrested for Medicaid Fraud, with Dr. Roy Shelburne

Dr. Roy Shelburne joins us on this episode to tell his incredible story about being arrested for Medicaid fraud. After years of running a busy and successful practice in Virginia, Dr. Shelburne received a phone call that the FBI had raided his office. All of his personal assets were seized, and he was tried and found guilty of medicaid fraud. He served 19 months in prison, and now spends his time consulting dentists on how to prevent this from happening to them.

This was an eye-opening conversation. As pediatric dentists, many of us see medicaid. You may think that if there were an error or mistake with an office's Medicaid billing, penalties and fines could occur and simply be paid as long as the mistake was innocent. Dr. Shelburne's story demonstrates that this isn't always true.

His education began with the study of Engineering at the University of Wisconsin, a degree in Mathematics followed by a Doctor of Dental Surgery and a Master’s Degree in Pediatric Dentistry from Indiana University.

In 1970, he joined the original Dental School Faculty at the University of Connecticut Health Center. He began his private practice in 1972, which has grown into a group practice consisting of five Pediatric Dentists and two Orthodontists. For more than forty years the primary focus of this practice has been preventive care with an emphasis on active child and parent participation.

I Walked Away From Pediatric Dentistry

It doesn't take long reading iPedo posts and talking to other dentists to realize that burnout, stress, and difficulty finding work-life balance is a common problem among pediatric dentists. In this episode, Dr. Natalie Datien joins us to discuss her story about why she walked away from a busy practice.

Dr. Natalie bought a practice shortly out of residency with her husband, a general dentist in the Detroit area of Michigan. Together they grew the practice from an old-school, low-production office into a multi-doctor GP practice with Dr. Natalie running the pediatric side of things. They were seeing over 100+ new patients a month and booking out for weeks. After three years of sleepless nights, staffing issues, worrying about cash flow, and dealing with difficult families, Dr. Natalie and her husband set down the handpiece and sold the practice. She is now retired at 33 and happily spending time with her young family.

We have a really interesting conversation about what life is like after dentistry, reflections on what could have made things easier as a practice owner, advice for young start-up docs, and just in general what type of things burn us out as practice owners.

Ortho-Peds Talk, with Dr. Gena Otto

Dr. Gena Otto is an orthodontist and the owner of Otto Orthodontics in St. Louis, Missouri. Her office has grown tremendously to include three locations, 30+ staff, and a full time marketing specialist. 

I wanted to have Dr. Otto on the podcast because we work closely together professionally, and I've always loved her hip and energetic enthusiasm for orthodontics. She has a fantastic social media presence, along with really nice clinical outcomes and a passion for what she does. 

In this podcast, Dr. Otto and I discuss some of the following ortho-peds issues:

Verbal Judo and Ridiculous Parent Moments, with Dr. Peter Nguyen

Dr. Peter Nguyen is a pediatric dentist who started First Bite Pediatric Dentistry in Henderson, Nevada. Dr. Nguyen grew up as the child of Vietnamese war refugees. He did his pediatric residency at UCLA, before moving to the Las Vegas area and associating for a number of practices prior to doing his start-up.

Dr. Nguyen has a great sense of humor and as he puts it, "I'm not the smartest guy on iPedo so I have to make up for it with my sense of humor." I loved this podcast, I thought it was awesome to just laugh out loud and tell stories about ridiculous things that happen in our practice. We talk about Dr. Nguyen's start-up journey, then discuss crazy and ridiculous things parents and kids have done in our offices. 

We also talk about "verbal judo" and the art of communicating with parents when it comes to tough treatment plans or difficult situations.

Starting a Practice with Your Spouse, with Dr. Cyrus Safizadeh

Dr. Cyrus Safizadeh is a pediatric dentist who opened a practice with his wife, a general dentist, shortly before COVID. They grew this practice together throughout the pandemic challenges to come out thriving on the other side.

Dr. Cyrus talks about the benefits of working closely with a general dentist and utilizing the skillsets and technology that his wife brings to the practice. He shares how they have balanced Medicaid patients to keep a healthy new patient flow on both the pediatric and adult side. He is kind of enough to open up and talk freely about his practice numbers and how he plans for growth. 

We also discuss how they made a 150k investment in a CAD-CAM unit, and Dr. Cyrus has ended up using it extensively in pediatric dentistry. After being exposed to so much general dentistry, he also talks about why he no longer starts sweating at the site of extensive permanent tooth decay and treatment plans.

Inside the World of Selling to a DSO, with Chip Fichtner of Large Practice Sales

Chip Fichtner is the founder of Large Practice Sales (LPS), a firm that specializes in partnering with large dental practices as they look to sell to DSOs. You may recognize him from the flyers and newsletters that his company mails out to private practices. Chip discusses how his company is able to achieve the highest EBITDA multiples by bidding practices to multiple DSOs to find the right fit. He also talks about "invisible" DSOs that allow the owner to retain full autonomy of the practice while still being able to cash out on a portion of practice equity. Chip likes packaging these deals as a "dental trifecta" to include pediatric dentists, orthodontists, and oral surgeons in the same geographic area to make for higher offers from DSOs. Some other questions I ask Chip:

- How do pediatric dentists obtain the highest value for their practices?
- Where is all this private equity money coming from?
- As part of these buyouts, we often see DSOs offer shares of equity in the parent company. Is there ever a concern about the value and solvency of these shares?
- How valuable are good pediatric offices, and what multiples are you typically seeing?
- Are DSO buyouts here to stay? Or is this a short-term process that will likely end in the future?

Building a Full Time Pediatric Surgery Center, with Dr. Jason Bresler

Dr. Jason Bresler is a pediatric dentist practicing out of the Philadelphia, Pennsylvania area. His father originally started and grew the practice in 1982, and now Jason and his siblings and their spouses (5 Dr. Brasslers in total) own and operate this large practice.  They have grown the practice to include eight pediatric offices, four orthodontic offices, over 30 associate dentists, 250 staff members, and along the way built their own surgery center. This stand-alone surgery center operates five days a week, with a full time nursing staff, pediatric anesthesiologists, and CRNAs. This allows two dentists to see 15 or more GA cases every day. 

We spent the majority of our conversation discussing how to set up a surgery center, regulatory differences, surgery center fees, and how a practice of this size is structured and operated. We also talk about what makes for a good associate pediatric dentist.

Busting Common Radiation Myths in Pediatric Dentistry, with Dr. Juan Yepes

Dr. Juan Yepes is a man who needs no introduction. He is a pediatric dentist, radiologist, physician, masters of public health... I'm sure I'm forgetting something. Let's just say he's smarter than I am. Dr. Yepes joins us on the podcast to talk about radiation safety in pediatric dentistry. 

We had a great conversation highlighting the importance of being responsible with how much radiation we are prescribing in the office. Some topics we discuss:

- Modern digital sensors vs traditional film x-rays, and why digital has its drawbacks
- Rectangular beam collimation and why we need to consider it

Partnerships, Associateship Challenges, and a Transition Story, with Dr. Mitch Kramer

Dr. Mitch Kramer is a pediatric dentist, partner, and soon to be 100% owner of a busy pediatric practice in central Minnesota. Dr. Kramer comes on the podcast to share his story about how he structured his initial buy-in in 2016. After helping grow the practice to four-locations, an associate, and 12,000 active patients, Dr. Kramer will be transitioning towards buying out the remaining practice equity from his partner Dr. Dennis Carreras this year. 

It was great catching up with Dr. Kramer, as this was the office that I associated with between residency and the start of the COVID pandemic. We were able to reflect on our opportunity to work together, along with the challenges of finding and retaining both good staff members as well as associate pediatric dentists. We talk about the financial practice opportunities that can be found outside of the urban setting, how to keep team members motivated, when can you afford another dental assistant, and batting clean-up for restorative work from previous associates that doesn't go as planned.

Dr. Tim Richardson is a pediatric dentist and the owner of Acorn Dentistry for Children in the greater Portland, Oregon area. After residency, Dr. Richardson bought and grew three offices in a five year period. Smokey Mountain Sedations, with Dr. Adam Priestwood

Dr. Adam Priestwood is a pediatric dentist who practices in eastern Tennessee and treats a high-risk population. Dr. Priestwood comes on the podcast to talk about how he uses oral sedations to help manage shortages with his access to OR time. Don't Do Sealants that Suck

This if the first of several solo study episodes where I will be reviewing certain pediatric dentistry topics as a way to review for board exams, as well as to review new evidence and clinical studies on the subject.

Today's study topic is on sealants... how to do sealants that don't suck. Placing a good retentive sealant is one of the most technically challenging procedures to do well, and yet we don't critically evaluate how we place them often enough. There's a high degree of bias with how we evaluate our own sealants. We all know that a lot of the previously placed sealants we see come into our offices look anywhere from rough, to partially retained, to recurrent decay garbage. In this episode I review both the literature and my personal thoughts on the following:

- AAPD clinical recommendations for sealants
- Sealing over decay
- Glass ionomer sealants and why you should consider using them
- Air abrasion. The evidence is so strong for it, why does nobody use it?
- Should we be bonding our sealants?
- Isolite and dry shield vs rubber dam vs cotton roll sealants
- Brands of sealant material
- Sealing primary molars

How to See 70 Hygiene Patients a Day, with Dr. Eric teDuits

Dr. Eric teDuits is a pediatric dentist and a private practice owner in Madison, Wisconsin. He has been growing and expanding his practice for nearly 30 years, and he joins us today for a great discussion on how to maximize efficiency and productivity in the hygiene department. 

Dr. teDuits discusses how he expanded and structured his business from one pediatric dentist to six partners with three different locations. We discuss how he brings on associates in his practice, structures buyouts, and involves the real estate. We transition into the focus of our conversation to learn more about how Dr. teDuits is able to see 60-70 recall patients in a day, with a focus on providing quality oral hygiene instruction while implementing good systems and a high-performance team to deliver the care. 

We wrap up our conversation by getting Dr. teDuit's thoughts on the future of private equity and the influence of DSOs in the pediatric dental space. This was a dense and very fluid conversation, and we touch on a lot of different topics besides just improving practice recare.

KPIs That You Should Be Tracking (But Probably Aren't), with Dr. Eric Hans

McDonalds knows the cost of a sesame seed on every bun they sell, yet when you ask a dentist what their case acceptance rate/monthly breakeven amount/recall retention rate of their practice is, most have no clue. With 2021 having come to a close, now is a great time to sit down over a cold beer or a stiff whiskey and start making some spreadsheets to understand the numbers in the business you are running. 

Dr. Eric Hans is a private practice pediatric dentist with nearly 25 years of business experience. We have a great conversation about important KPIs (key performance indicators) that dentists should be watching in their practice. Dr. Hans really emphasized in our discussion that building a strong relationship with both your team and the patients in your practice is the foundation to why we need KPIs in the first place. 

This was a challenging topic to cover because it can be so broad, but we did a nice job touching on most aspects of it. Everybody knows that a practice owner should monitor production, collection, new patient numbers, overhead, A/R, etc. But many KPIs go a lot deeper than these numbers and are critical for keeping a pulse on how things are going in your practice. 

Some of the things we talk about in our conversation:
- Tracking your hygiene department, daily hygiene numbers, and restorative to hygiene ratios
- Breakeven costs and when are you losing money to do dentistry
- Production on GA/sedation days vs clinic days
- Setting goals and how often to track them
- Monitoring new patient flow, age of your patient population, and knowing how to expand your capacity
- Watching your P&L, and when is it ok for your overhead to be higher

Is SDF Being Abused?

Dr. David Bailey is a private practice pediatric dentist in Sheridan, Wyoming. He has been using SDF in his practice for years, and was an early advocate for using it as a caries-arresting medication. While still an important component of his practice, he has since limited SDF to specific patient situations. 

Dr. Bailey and I have a discussion about the potential issues with widespread SDF use in our profession, in light of recent issues regarding non-coverage by insurance companies for restorations placed on teeth previously treated with SDF. We discuss the pros and cons of its use, along with how the esthetic drawbacks can be perceived by the public.

We go over six different unique patient situations in which SDF may or may not be appropriate compared to more traditional means. This was a great exercise for identifying situations in which SDF would be the perfect first-line treatment option, while in others it may do more harm than good.

If I've learned anything about dentists, it's that they are resistant to change and easily get stuck in doing things the way they've always done them. A good dentist recognizes his or her own shortcomings, and isn't afraid to try new procedures and materials, and step beyond the edge of the comfort zone. 

Dr. Sean Whalen is a seasoned pediatric dentist and practice owner from Westminster, Colorado. We discuss different techniques, procedures, materials, and ideologies that a well-rounded pediatric dentist should be offering. Dr. Whalen is passionate about providing a full range of services to best serve our patients, including interceptive ortho, oral conscious sedation, oral surgery, posterior zirconia, advanced behavior management and many others. One Million Dollars in Student Loans (Part 3 of Soul-Crushing Debt)

This is the this third and final segment of our giant student loan story-time.

Dr. Jared Johnson is a pediatric dentist and the owner of Arctic Dental in Muscatine, Iowa. He started his practice from scratch along with buying the practice building, buying a home, and piling all of these things on top of his large student loan debt. He was able to aggressively pay down debt by associating one week per month at a profitable associateship out-of-state, all while taking advantage of the current federal student loan interest freeze to pay down $8000 per month towards principal.

Most of us leveraged some degree of student loans throughout our extensive education process, and these loans influence the financial decisions we make every day.

5 Lessons Learned After One Year of Owning a Pediatric Practice

It's been 12 full months since we opened the doors of my startup practice here in Missouri, and we had a fantastic (but challenging) first year. I wanted to take the opportunity to do a bonus podcast and review some of the things I've learned after one full year of starting and running a busy pediatric dental practice.

Cuivre Creek Pediatric Dentistry opened last October during the COVID-19 era, and in that time our team has a built a practice that has rapidly expanded to averaging 160+ new patients per month. We've had a lot of growing pains with such a high demand for our services, and I'd like to share some of the mistakes and lessons learned that might be able to help out young practice owners.

200+ New Patients a Month, with Dr. Kevin Kennedy

Dr. Kevin Kennedy is a pediatric dentist and the owner of Spring Creek Pediatric Dentistry in Clarksville, Tennessee. Dr. Kennedy's scratch-start practice is only 2 years old, but it has grown to a point where he is averaging 50 patient visits per day and a new patient flow of 200-250 per month. 

We have a great discussion on a variety of topics, including challenges of practicing in a military town, aggressive marketing budgets, knowing your numbers, and what Kevin's game plan is for when he hits his office capacity. Coaching and Consulting in Pediatric Dentistry, with Dr. Robert Elliott

When Endo and Pediatric Dentistry Collide, with Dr. Ted Beauchamp

Dr. Ted Beauchamp is an endodontist and the owner of Beacon Endodontics in Wentzville, Missouri. Dr. Ted served in the Army after dental school, where he completed a one-year Advanced Education in General Dentistry (AEGD) residency program, then received his certificate in Endodontics after an additional two-year residency. Dr. Beauchamp continued to serve his country, practicing in South Korea, and then at Fort Campbell, Kentucky, as Chief of Endodontics. He mentored and taught endodontics to new general dentists and oversaw twelve dentists and forty support staff on base.

Leaving the Army, he entered into private practice with a well-respected endodontics office in St. Louis, then started Beacon Endodontics in 2020. He and Dr. Casey grab a beer at a local brewery and talk about some of the tough endo decisions that pediatric dentists face. 

We discuss using MTA, permanent tooth pulpotomies vs pulp caps, when to leave decay, long-term outcomes of apexogenesis as definitive treatment, when to initiate root canal therapy after trauma, and a host of other topics.

General Dentist Owned Pediatric Dental Offices, with Dr. Chad Carpenter

Dr. Chad Carpenter is a general dentist practicing out of Rapid City, South Dakota. Dr. Carpenter identified that there was a substantial access to care problem for high-risk kids in his population. He took matters into his own hands and started a general-anesthesia heavy pediatric dental office, employing both general dentists and pediatric dentists. His unique situation as a GP owner who employs pediatric dentists allows him to provide a unique perspective on dental care in a rural population, specifically a heavy Native American population. 

Dr. Carpenter and I discuss how he runs both a GP office and a pedo office, challenges and benefits of treating Medicaid, and how he provides quality dentistry for these kids in his office.

The Evolution of Pediatric Dentistry, with Dr. Mike Kanellis

We have a very special guest for today's episode. Dr. Mike Kanellis has been practicing pediatric dentistry for over 40 years. Following his pediatric training at the University of Iowa, Dr. Kanellis started and operated a private practice in Muscatine, Iowa for 13 years. He then sold his office and made the transition into the world of academics and now acts as the associate dean for patient care at the University of Iowa College of Dentistry. He continues to actively see pediatric patients at the dental school.

Dr. Kanellis shares a wealth of knowledge about his experience in pediatric dentistry. We talk about what it was like seeing 30-40 patients per day out of his original 400 sq/ft office. Dr. Kanellis has always been interested in outside-the-box thinking to manage decay, and he discusses his use of Hall crowns 30 years before it was an established technique. Dr. Brian Richards is a private practice pediatric dentist who joins us to discuss how he makes his practice successful using old-school techniques that work for him. Brian has posted several of these examples online showing traditional operative work being completed on 2, 3, and 4 year-olds using rubber dam isolation, behavior management, and no laughing gas or sedation.
